Online concussion training is now available. Please staple the certificate to your physical. Write your sport in the upper right hand corner and turn the paperwork into the Activities Office to Mr. Mobley or Mrs. Miller ONLY. This will minimize the handling of paperwork. Volleyball players should have paperwork turned in by 8/1.
FOREST PARK H.S. MIDDLE SCHOOL CAMP
The FPHS volleyball clinic will teach fundamental skills like serving, passing, setting, attacking, defense, and game play. The clinic is open to all girls ages 10-14 and in grades 5th-8th during the 2014-15 school year.
The clinic will be taught by FPHS head volleyball coach, Anna Davila. Coach Davila has been a head volleyball coach for 10 years and will be starting her 5th season with the Bruins. Coach Davila is a Prince William County certified coach. She has a CAP I certification with USA Volleyball and will be completing a master’s degree in coaching and athletic administration from Concordia University this fall. Forest Park Volleyball assistant coaches and 2013 varsity team members will be assisting with the clinic.
The clinic will be held July 21-24 from 4:00-7:00 p.m.
All athletes should wear shorts, t-shirts, knee pads and indoor court shoes.
Athletes should bring plenty of water and/or sports drinks and a snack.
There will be a short snack break at around 5:30 every evening.
NEW NCAA Division I Initial-Eligibility Academic Requirements
There are new requirements for college-bound student-athletes enrolling full time at an NCAA Division I college or university on or after August 1, 2016.

2014 Fall Tryouts & Practices
The 2014 Varsity and Junior Varsity tryouts will be held from August 4-7, 9:00a.m.-12:00p.m. The first day of practice will be August 8, 2014 and then every weekday throughout the season. Any athlete that would like to tryout for a team must be present at the tryouts and all practices.
The Freshmen tryouts will run August 6-11, 9:00a.m.-12:00p.m. The first day of practice will being August 12, 2014 and continue every weekday throughout the season. Any athlete that would like to tryout for a team must be present at the tryouts and attend all practices.
THESE ITEMS ARE MANDATORY FOR ALL ATHLETES TO TRY OUT:
1. CURRENT SPORTS PHYSICAL, dated after May 2014
2. CONCUSSION TRAINING, anyone competing this for the first time must attend the "in person"
training. Both the athlete and one parent/guardian must attend the training.
3. EMERGENCY BLUE CARD - this can be attained in the front office
* You may turn in the physical to the front office at any time or bring it with you on the first day of tryouts. The concussion trainings will be tracked by the school's athletic trainer. The emergency blue card should be filled out and signed by a parent our guardian prior to the first tryout. This card should be given to the coach checking you in on your first tryout date.

August 8 - 9a.m.-12p.m
August 11-15 - 9-11a.m. and 5-7p.m
August 18-28 - 9a.m.-12p.m

*Mandatory Concussion Educaton Training*
Parents and student athletes must take concussion education training before sports tryouts. All student athletes and at least one parent or guardian will be required to attend a concussion education session provided by their respective middle and high schools. Attendance at one of these sessions is required before a student will be allowed to try out for any sport. Participation in the concussion education session is mandated by state law (Virginia Code 22.1-271.5). A concussion is a brain injury that is characterized by an onset of impairment of cognitive and/or physical functioning, and is caused by a blow to the head, face, or neck, or to the body that causes a sudden jarring of the head (e.g., a helmet to the head, being
knocked to the ground). A concussion can occur with or without loss of consciousness.
Prince William County Public Schools supports the need to ensure that its staff, students, and the community are aware of the seriousness of brain injuries and concussions and how they can affect the student’s
abilities in the educational setting. In April 2011, the Prince William County School Board adopted a concussion policy that meets the Virginia Code. The law requires parents and students to sign a form attesting that they received training prior to any level of student participation in athletics. Athletes should attend a session at the high school they attend. The meeting will start promptly at 7:00 p.m. and no one will be allowed to enter once a session has started.

In an effort to support the FP Booster Club, it is strongly encouraged that all members of the volleyball program have a parent join as a member of the booster club. The cost is only $5.
